If you have used VA Benefits at another institution:
In addition to the Veteran Semester Enrollment Form and a copy of your DD-214 or Notice of Basic Eligibility (NOBE), students transferring from another university should also submit:
- Request for Change of Place of Training (VA Form 22-1995)
- Request for Change of Place of Training (VA Form 22-5495) Chapter 35 Dependents only
- A credit evaluation must be completed for undergraduates by the Offices of Admission
